The flat is the broad section of cartilage between the helix rim and the inner ear. As the name suggests, it sits flat against the side of the head, which makes it one of the most versatile placements in the ear. The flat supports both single pieces and patterns of two, three, or four piercings working together as a constellation.
At Piercing Lab Calgary, every flat piercing is performed with a sterile single use needle and fitted with implant grade titanium or solid gold from the first appointment.
Why fit and styling matter
The flat is the placement designed for pattern. Unlike helix where pieces stack along a curve, or tragus where one stud stands alone, the flat invites geometric arrangement: a paired set, a triangle, a curved line, or a small constellation. Four considerations shape every flat fitting:
Bar length. The flat is thicker cartilage than the tragus but flatter than the conch. Initial bars accommodate 4 to 8 weeks of swelling, then downsize for clean wear.
Spacing. When planning multiple flat piercings, spacing matters more than size. Pieces that sit too close compete with each other; pieces too far apart lose the constellation effect. We plan spacing during consultation, before any piercing.
Geometric clarity. Pieces with clean geometry (rounds, kites, trillions, princess cuts) read better in constellation layouts than baroque or organic shapes. Save sculptural pieces for solo flat wear.
Pattern logic. A flat constellation is designed top down: choose the largest anchor piece first, then plan smaller complementary pieces around it. Asymmetric arrangements often outperform perfect symmetry.
Jewelry material
We exclusively use implant grade titanium and solid gold suitable for long term wear and healing cartilage piercings.
Implant grade titanium (ASTM F136). The industry default for fresh piercings. Hypoallergenic and proven biocompatible. Anodised colour finishes remain safe in healing tissue.
Solid 14K or 18K gold. Nickel free alloyed gold for both fresh and healed piercings. Niori Jewelry, our in house line, is solid 14K gold across the full range.
Fashion jewelry materials are not used for new piercings at our studio.
Placement variations
The flat has two common configurations:
Single flat. One deliberate piece, typically larger than tragus jewelry but smaller than a conch centerpiece. Works as a focal point or as part of a future curated ear.
Flat constellation. Two, three, or four piercings on the same flat, planned as a coordinated pattern. Most often used to create a triangle, diamond, or curved arc of small studs.
We assess your flat anatomy before any piercing decision. Spacing for constellations requires enough cartilage real estate to support multiple piercings without compromising healing.
Healing and aftercare
Flat piercings need 6 to 12 months to fully heal. The outside may look settled before the inside has finished, so aftercare runs the full window.
First 8 weeks. Saline rinse twice daily. Avoid sleeping directly on the piercing. Hair, masks, and clothing that touches the upper ear can catch new flat piercings during early healing.
Weeks 4 to 8. Free downsize appointment with any Niori Jewelry purchase. Flat piercings benefit from downsizing especially in constellations where multiple bars sitting close together can otherwise feel crowded.
Months 3 to 12. Continue saline rinses as the inside of the piercing continues to remodel.

Should constellations be symmetric or asymmetric?
Both work, but asymmetric arrangements often look more natural and modern. Perfect symmetry can read as static. We sketch the layout on your ear with a marker before any piercing so you can see the planned pattern in advance.
What jewelry sizes pair well in a flat constellation?
Coordinated rather than identical. A common approach is an anchor piece around 3mm to 4mm with smaller 1.5mm to 2mm complements. Stones of the same cut family (all round, or all kite) tend to read more cohesive than mixed shapes.
How is the flat different from the helix?
The helix curves along the outer rim of the ear, while the flat sits inside the rim as a broad flat surface. The flat is better for constellation patterns because pieces sit on a single plane; the helix is better for stacking because pieces follow the curve.
Do you offer consultation before a flat piercing?
Yes, and for any planned constellation we require it. Spacing decisions need to be made on your actual anatomy rather than from photos, and a consultation lets us plan the full pattern before committing to the first piercing.
Can I bring my own jewelry for a new flat piercing?
For healed flat piercings we install outside jewelry that meets our material standards. For new piercings we use only studio supplied implant grade titanium or solid gold.
